- We will send it to you through email.
- Download the attached file. The password of the file is your birthday in MMDDYYYY format, for example, '12011990'.
- Print seven (7) sets of the document in the legal-size bond paper.
- Each page in separate sheets.
- 2 pages x 7 sets = 14 pages total. - Affix your signatures on the left side of the 1st page, on the 2nd and last page kindly sign where your name indicates.
- Please do not sign anything under 'Signed In The Presence Of' on the last page as this is for notary purposes.
- Double-check to be sure there are no erasures, stain marks, or inkblots in the printed sheets. Please also be sure all pages are signed, intact, and complete before mailing them out as we will not accept printed sets with errors.
